How LDL Cholesterol Is Calculated (Friedewald)

Most labs estimate LDL rather than measuring it directly, using the Friedewald equation. The triglyceride term approximates VLDL cholesterol:

mmol/L LDL = Total cholesterol − HDL − (Triglycerides ÷ 2.2)

It is quick and free but only an estimate. It is unreliable when triglycerides exceed 400 mg/dL (about 4.5 mmol/L), in non-fasting samples, and at very low LDL. The newer Martin-Hopkins method uses an adjustable factor instead of a fixed ÷5 and is more accurate in those situations; many labs now report it.

Example LDL = 200 − 50 − (150 ÷ 5) = 120 mg/dL

120 mg/dL is in the near-optimal range. In mmol/L, that LDL is 120 ÷ 38.67 ≈ 3.1 mmol/L.
