Why are hormones reported in so many different units?

Each hormone has a different molecular weight and is present at very different concentrations, so labs pick units that suit the scale — ng/dL, pg/mL, µg/dL or pmol/L. Converting to the unit your reference range uses is what makes the value interpretable.

Does the timing of a hormone test matter?

Often, yes. Cortisol follows a daily rhythm and is usually drawn in the morning, testosterone peaks in the morning, and estradiol and progesterone vary across the menstrual cycle. The draw time or cycle day is part of interpreting the result.

What is the difference between total and free hormone levels?

Total measures hormone bound to carrier proteins plus the small free fraction; free (or bioavailable) is the active part. When binding proteins are abnormal, the free level gives a clearer picture — as with free testosterone or free thyroid hormones.
